The technique of the dilatation of the male sexual organ is somewhat recent in relation to the technique of elongation, so that first papers date from the end of 80s of XX century (Samitier, Reed). There are two basic methods, and these are Dermal Fat Graft (DFG) and Autologus Fat Transfer (AFT).

In the process of the first method, a part of the skin area is cut out from the rim of the backside and prepared for implantation under the skin of the penis on the inner membrane Tunica Dartos. The approach is like that of through the cut in trimming (circumcision).

The other technique is the transplantation of fat tissue, which is nowadays a more acceptable technique.

Pre-operational development

Pubic area has to be shaved, and laboratory tests of blood and urine samples, as well as the ECG, should be done pre-operatively. For this type of the operation of fat tissue the patient should be circumcised, in other words the surgical treatment should be done 6 - 8 weeks before the operation of dilatation where fat tissue is used.
 
Anaesthesia

The surgical treatment is usually carried out under the spinal anaesthesia, but se could also be performed under the general endotracheal anaesthesia.
 

Transplantation of fat tissue into the inner membrane of the sexual organ

The operation

When transplanting fat tissue, before all the region from where fat tissue could be taken is chosen and marked, in other words from the predilection area, for its amassing at men.
This is usually the pubic area immediately above the root of the penis or the lower part of the abdominal wall itself. After the preparations have been done, fat tissue is removed using negative pressure from the chosen area, and then it is processed and squirted into penis membranes with an injection. It is injected into the area of where the circumcision (trimming) has been performed, so, this wound from the previous surgical treatment therefore has to be completely healed up.

After the operation around the sexual organ, a bandage is applied, and then it is bandaged with dressing. In respect of the fact that in spite of good preparatory treatment of the transplanted fat tissue, it partially undergoes the process of necrosis, and is partially absorbed by the organism, while some quantity of it even calcificates, so that the final result is that there remains about 60 – 70 percent of the original transplanted fat tissue.

Post-operational development

After the operation, during the next 24 hours, re-dressing and control of the lab results of the local area that has been operated on is carried out. Bandage is kept on the genitals for about 8 – 10 days. Antibiotics are obligatory for a week, and Diazepam 10 mg to be applied in the evening, for the first period after the operation, because nighttime-erections could be painful. Continence is advisable for the period of 6 weeks after the operation. Stay at the medical institution, under qualified supervision and care, takes 24 hours after the surgical treatment. Locomotion is also limited during the few days after the operation.

Complications

Infections and inappropriate redeployment of fat tissue around the penis can happen. The scar is almost invisible because it is located at the spot of the formerly done circumcision. If the result is inadequate, the procedure can be repeated in the period from 6 months to 1 year.

Annotation

Some authors that is operators, combine the operation of elongation and dilatation as one surgical treatment, but complications are then more frequent.
